Checks should be written out to Grant Meenach.
Logo wear orders will not be made until all payments have been received.
Sweatshirts are $25/shirt
T-shirts are $20/shirt

Order Form:
Some students have expressed an interested in sweatshirts with the MDK-Crestwood logo.
To determine how much interested there is for sweatshirts and/or additional t-shirts, please fill out the form below. We ask that all ‘orders’ are in by February 10 to determine if there is enough interest to create an order with our t-shirt vendor.
If so, we will notify you via text that payment is due by class on February 13th. We will need all payments before we can proceed with the order. Master Meenach will use the money collected to pay when placing the order.
Items will be distributed in class once the order is ready.
